El objetivo de este libro es el tratamiento de esas dos formas de la neurosis que son la histeria y la obsesión. En el primer capítulo se expone una reflexión general acerca del ser neurótico. En los dos capítulos siguientes se presentan los elementos capitales para el diagnóstico y el inicio del análisis. En el último capítulo se retoman consideraciones en torno a la dirección de la cura y la posición del analista. La virtud principal de este libro radica en que no presupone aquello que debe explicar. Por eso es que junto al esclarecimiento de conceptos se presentan diversos fragmentos clínicos dirigidos a transmitir la teoría –para no recaer en la especulación o el dogma–. Así, por ejemplo, es que los dos capítulos centrales realizan una relectura de dos célebres casos freudianos (el caso Dora y el Hombre de las ratas) para demostrar el carácter paradigmático que les corresponde. De este modo, tiene el lector en sus manos un libro de inspiración lacaniana que se caracteriza, entonces, por su retorno a Freud.
